Kelis received the song, which was written and produced by the Neptunes, instead of being offered to Britney Spears for her fourth studio album, In the Zone.

The song's use of the term "milkshake" as a metaphor for "something that makes women special" is explained by Kelis.

When Tasty was selected as the album's title, the song was born. Kelis chose the song to be the first single off the album as she "knew right away that it was a really good song" when she was making it.

Kelis responded to The Observer when asked what she thought the song meant, saying, "It means whatever people want it to; it was just a word we came up with on a whim, but then the song took on a life of its own." But she compared a person's milkshake to their confidence in an Associated Press interview, saying, "A milkshake is what makes women special." It's what makes us exciting and gives us our confidence."

2003 saw "Milkshake" reach its highest position on the US Billboard Hot 100, at number three. The song also peaked inside the top 10 charts in Australia, Denmark, the Netherlands, New Zealand, Norway, Sweden, and the United Kingdom.
